Unio’s CCM program goes beyond traditional chronic condition management to offer:
- A dedicated patient navigator for symptom triaging, general health questions, and coordinating appointments, refills, labs, and community support services;
- Cystitis, OAB, prostate and bladder cancer, BPH, IBD, and fatty liver care coordination and support;
- Ongoing health management with structured regular check-ins;
- Reduced hospitalizations and ER visits through proactive, preventative care; and
- Increased access to personalized care over the phone.

A Patient-Centered Approach

Who Qualifies?
Unio’s CCM services are covered by Medicare and most commercial insurance plans, making them widely accessible. Patients with at least two medical conditions expected to last at least six months may be recommended for CCM by their provider.
